Profile
I have always been a passionate advocate for the clients I work with to ensure their voice is heard and their choices and wishes respected. I have a positive attitude to what my clients can do and enjoy working with a wide range of professionals and services to enable my clients to achieve their goals.
I have had many years of experience working across health and social care with clients with a range of disabilities and their families, and as a Case Manager working for ILS, I am committed to ensuring I provide an excellent service for all of my clients.
Previous Clinical Experience
April 2011- August 2024
Cheshire West and Chester Council – Social Worker
- Assessment, co-ordination and arranging services for clients. Completing regular statutory care act assessments, mental capacity assessments, continuing health care assessments and best interest meetings. Experience of working with clients with complex physical and mental health needs working with therapy, nursing and reablement teams.
- Case management experience of those requiring adult social care input in the community and /or placements. My role involved managing risk and ensuring plans are in place to promote well-being, safeguard and support people experiencing abuse/neglect.
April 2009 -April 2011
Social Worker, Rehab Link Team, Cheshire West & Chester Council
- A case management role within a rehab team consisting of a Physiotherapist, Nurse, Occupational therapist, Psychologist and two Social Workers (myself included). I led on assessments both in the home and within intermediate care beds in the community. The role focused on the patients rehab and goal setting, signposting and advocating for what they need to enhance positive rehab outcomes and how best to achieve this.
- Having worked in rehab intermediate care I have seen first hand the impact of good quality case management and the importance of creating effective and enabling environments for injured people and their families
April 2008
Social Worker, Cheshire East Council
- The post within the learning disability team involved case management of complex high cost care packages working with adults with disabilities. I demonstrated expert and effective social work practice, assessed and managed risk and built rapport with families and clients. The post involved supporting those with learning disabilities in making choices, finding employment and promoting independence.
- This role also involved lots of co-ordinating / planning of transitional cases moving from children’s services to adult social care. The social worker post covered a wide range of needs from social care, employment and health and wellbeing.
